For your convenience, you may book an appointment online. If you are booking your initial evaluation, you will be prompted to fill out some intake forms. It may be helpful to use your phone or tablet to make it easier to upload a picture of your insurance card. This is important so we may verify your benefits before your appointment.
If you are scheduling an appointment for trigger point needling or are interested in adding trigger point needling to your treatment (your therapist will determine whether it is appropriate for your condition), please fill out the consent form and bring it with you to your appointment. Thank you.
